A Medical Record will be created during your first visit to SKMC. To maintain your confidentiality, SKMC will not share any of your medical information except to those whom you mention in your Patient Confidentiality Form. You can identify two relatives or friends who may request and receive your medical reports in the future.
If this form is not completed, or in the absence of any written authorization from you, you will be the only person able to request and receive your medical reports. Access will be denied to other family members or relatives, as per SKMC policy.
Please note the following when requesting your medical information:
Patients 18 years and over:
You must submit your request in person or send a representative with a written authorization, signed and dated by the patient.
You must provide a copy of the identification card of both the patient and the representative.
Patient under 18 years:
To maintain your privacy and confidentiality, no photocopies will be given without written authorization from the patient. You may receive photocopies of any document from your file by following the instructions below:
- If you require a typed report such as a Medical Evaluation, Consultation Report, Operative Reports, Discharge Summaries etc., then you must request this from your doctor during your appointment. The report should be ready within five working days.
- If you require a Medical Evaluation Report to seek medical treatment abroad (i.e. International Affairs Division), then then you must request this from your doctor during your appointment. The report should be ready within ten working days.
- If you require copies of other documents (e.g. lab results, x-ray results, handwritten progress notes etc.) please request these from the Health Information Management Services. These documents should be ready within 48 hours of your request.
- X-Ray films can be taken from the Radiology Department.
- Copies of complete charts will take a minimum of 72 hours.

The patient or authorized person may request copies of medical reports from the Release of Information window in Health Information Management Services, located on the Ground Floor of the Out Patient Specialty Clinic (OPSC). They will be requested to complete a Fee for Release of Information Form before the reports are released.
- The form is taken to the Cashier at the Information Desk, located on the ground floor of the OPSC.
- The Cashier receives the payment and stamps the Fee for Release of Information Form.
- The stamped form is then handed back to the ROI Clerk at the Release of Information window, who will provide copies of the requested reports.
- The patient or authorized person must go to the Release of Information (ROI) window and fill out a Patient Request Report form. They will take the completed form to the Cashier in the OPSC to make the payment.
- The patient will return the ROI Form after being stamped “Paid” by the Cashier.
- The ROI staff will deliver the request to the Charge Nurse in the Clinic for the Physician to dictate the report.
