The Home Care Program cares for 420 patients discharged from hospital who have mobility problems and are willing to accept care at home. Patient age in the Home Care Program ranges from newborn children to those in the twilight of their lives. Service is provided by a Consultant Family Physician, a Specialist Physician with specialization in Cardiology, and a cadre of well-trained Nurses in addition to Respiratory Technicians, Dieticians and Occupational Therapists.
The Home Care Program team helps, for example, patients requiring tracheostomy care, enteral feeding advice, wound care, ‘activities of daily living’ assessments and home safety advice. It also manages anticoagulant therapy at home, diabetes and blood pressure management. Palliative care at home is provided. The Home Care Program also supports the Pediatrics Institute for hemophilia prophylaxis and others.
